Insurance approval required*** All non-insurance telehealth must be pre-approved and confirmed by provider/practitioner.
This appointment is designated specifically for only the questions and documents provided by Medicare to be answered and completed. Any other issues or questions must be discussed at a separate visit.
*Patients with insurance will receive a bill directly from the lab. Patients without insurance will pay Aspire Family Medical for the cost of labs and testing at the time of collection.
***It is the patient’s responsibility to verify with their insurance provider if they are eligible for Telehealth visits.

Labs Drawn at the Office
Fasting labs taken from 8 AM to 9 AM (last appointment time: 8:45 AM). Non-fasting labs can be drawn at any time. Billing varies*
